If you you need to cancel or reschedule an appointment you have a two options:

You can reach out to your clinician directly using the contact number they provided to you.

Or you can send your clinician a Message via your Therapy Appointment Client Portal. Below is the general link for the TA Client Portal where you can log in to send a Message.
​Cancellations must be received 48 hours prior to your appointment date and time. Otherwise, a No Show Fee may apply.

Medical Record Requests
​All medical records requests must be received in writing. If you need a copy of your medical record, ask your clinician to assign the Request for Medical Records Form to your TA account for completion. Once done, inform your clinician so they can notify the Records Custodian of your request.

Any request for a medical record received from another practice or physician must be received in writing (via fax or mail) and must contain a sign Release of Information from the client.
